Medication Authority Form Request

Use this form to request a form for school, OSCH, kindergarten etc. If you are asking for something different to the prescription, you will need an appointment. The cost is $15 per form not per medication BUT you MUST enter medication separately. The is NOT a request for a prescription. APHRA (the medical board) is very strict – no appointment = no script.
